Tyler Longwell 21787a66d0 docs: add TOP_OF_MIND.md working memory layer to nest spec 
- RAM layer between boot prompt (NEST.md) and disk index (CATALOG.md)
- Five sections: Current Focus, In Flight, Recent Decisions, Open Questions, Do Not Revisit
- Strict entry formats with date + source provenance on all entries
- Soft cap 120 lines, hard cap 200 — Builderclaw's failure was noise from bloat
- Update on state transitions mid-session, not just session end
- Single-writer rule: only primary session/orchestrator edits, delegates report via output
- Injected first in hook chain: TOP_OF_MIND → NEST.md → CATALOG.md
- Codex 9.3/10, Gemini: ship
