* UI polish: provider icons, dynamic placeholder, sidebar refinements
- Add provider icons (Goose, Claude Code, Codex) to the provider
selector dropdown and trigger button
- Make chat input placeholder dynamic based on selected provider
("Message Claude Code, @ to mention personas")
- Lighten placeholder text color and font weight
- Update toolbar button variant to use text-foreground
- Fix dropdown icon alignment for single-line vs multi-line items
- Hide sidebar scrollbar while preserving scroll functionality

Add group-focus-within:visible and group-focus-within:opacity-100
so the "New project" and item menu buttons are reachable via tab
navigation, not just mouse hover.
