Matt Toohey 1fb4429b42 fix: use real start time for tool call elapsed timer (#123) ...

Record the epoch timestamp when a tool call begins executing and pass it
through to the elapsed-time hook, so the timer reflects actual wall-clock
duration instead of resetting when the component re-mounts.
