Michael Neale dc370d3439 fix: gate tool-pair summarization to context windows <= 64K 
Tool-pair summarization causes stale ghost messages to be injected as
Role::User, leading to the agent re-engaging with resolved work, breaking
working builds, and trying non-existent CLI flags in long sessions.

On models with large context windows (128K+), three other mechanisms
already handle token pressure: tool output truncation (50 lines/50KB),
large response file redirect (>200K chars), and full compaction at 80%
context usage. Prompt caching further reduces the cost of keeping old
tool pairs around.

Gate summarization to context windows <= 64K, where truncation + compaction
alone may not provide enough headroom. This effectively disables it for
all modern models while preserving it for small-context models where it
may still help.

Users can still control via GOOSE_TOOL_PAIR_SUMMARIZATION=true/false.

Update integration test mock provider to use a 32K context limit so the
test exercises the feature under the new gate.
