spencrmartin 8ea6a9a0dc debug: re-add extensive logging to AgentManager provider configuration 
- Add detailed debugging to configure_default_provider method
- Add warnings when provider configuration fails
- Add debugging to get_or_create_agent provider setting
- This will help identify why the system is falling back to OpenAI
- Should reveal if Databricks provider configuration is failing
