jh-block 1fe6dd1dfb fix: scope delete lookup to apps extension and delete file before cache ...

- Include mcp_servers check in the find predicate so a URI collision
  from another extension can't match the wrong app.
- Delete the HTML file before the cache entry so a failed unlink
  doesn't leave the app missing from cache while still on disk.
